I declare that:

1. Auspro Group provided me with all the product information. I have chosen the product as per my requirement and the product is fit for purpose.

2. I am the energy consumer or I am authorised to make this declaration on behalf of the above entity that is the consumer.

3. I understand that I am the one who is responsible for disposing of the Freezer package rubbish (paper cartons, plastics, Styrofoam, etc.) and the installer will NOT take away any rubbish upon delivery or after installation.

4. To the best of my knowledge, the information provided by the installer in Section 2 is complete and accurate.

5. That the above activity was not undertaken at a scheduled activity premises listed on the Essential Services Commission’s Register of Scheduled Activity Premises on the VEU Registry. If undertaken at a scheduled activity premises I declare that the appropriate notification of intention to undertake prescribed activity has been made to the Essential Services Commission.

6. I understand that by signing this form I am assigning the right to create certificates for the installation to Auspro Group.

7. I have received an identifiable benefit from Auspro Group in exchange for assigning my rights to create the certificates for the above installation.

8. The Essential Services Commission has the right to inspect the installation with reasonable notice.

9. I understand that information on this form will be disclosed to the Essential Services Commission for the purpose of creating certificates under the Victorian Energy Efficiency Target Act 2007 and for related verification, audit and program monitoring purposes.

10. I understand that the Essential Services Commission may use the details provided on this form to issue commission correspondence. .

11. I am aware that it is an offence to provide false or misleading information to the Essential Services Commission or any person exercising powers under or in connection with the Victorian Energy Efficiency Target Act 2007.

12. I understand that if I let someone who is not from Auspro Group fix the unit or fix it without permission the warranty will be considered void.

13. By signing this form, I confirm that the information I have provided and all the agreements stated in the forms are hereby understood and accepted.

14. I authorise and agree to be contacted by Auspro Group and its subsidiaries regarding all the products and services from time to time.
